Bichectomy: Is It Worth Removing the Buccal Fat Pads?

Why are we writing this article?

Occasionally, during consultations, between routine check-ups, the question arises: “Doctor, what do you think about removing the buccal fat pads?”

Often, this curiosity is accompanied by a photo of a sculpted face or an influencer who has had a subtle “touch-up.” Behind this question lies a genuine concern for enhancing facial harmony.

Bichectomy has become one of those procedures that elicits as much interest as it does doubt. Therefore, today we want to discuss calmly and professionally what it is, when it might make sense, and when it’s better to think twice.

    What is bichectomy?

Bichectomy is a minimally invasive surgical procedure that involves removing the Bichat’s fat pads, natural accumulations of deep fat located in the cheeks. The most common aesthetic goal is to slim the face and define facial contours, especially in individuals with very rounded cheeks.

It is performed through small incisions inside the mouth, leaving no visible external scars, and recovery is typically quick.

    Potential benefits of bichectomy

    • Improved facial definition: By reducing volume in the mid-cheek area, the face can acquire a more sculpted appearance. In some cases, this helps accentuate the cheekbones and gives a “slimmer face” sensation.
    • Permanent results: Since fat tissue is permanently removed, the effects do not reverse as with temporary treatments (like dermal fillers).
    • Simple procedure with good recovery: Being intraoral, it usually involves less postoperative discomfort and a lower risk of external scars.

A study published in the Journal of Cosmetic and Laser Therapy concludes that bichectomy improves the aesthetic perception of the midface in appropriately selected patients.

But… is it for everyone? What to consider before deciding

    1. Each face has its anatomical logic Bichat’s fat pads are not “excess fat.” They fulfill biomechanical and aesthetic functions, and in many cases, their volume naturally decreases with age. Excessive or inadequately planned removal can prematurely age the face, accentuating grooves and bony structures.
    2. Your health first Like any surgical procedure, bichectomy requires prior medical evaluation. It’s important to review your general health status, habits, and rule out contraindications.
    3. Realistic expectations This procedure does not substitute for weight loss nor completely transforms facial proportions. It can subtly slim the face but will not drastically change your features. A facial aesthetics specialist is best suited to guide you on what is or isn’t achievable.

The Aesthetic Surgery Journal warns that inadequate patient selection and unrealistic expectations are key factors of dissatisfaction in procedures like bichectomy.

How to know if you are a good candidate

You might consider an aesthetic evaluation if:

    • You feel your face appears excessively rounded despite having a healthy body weight.
    • A qualified professional has assessed your facial proportions.
    • You seek a subtle improvement in facial harmony, not a radical change.

In a specialized consultation, we can conduct a comprehensive evaluation considering your facial anatomy, general health, age, and actual objectives. In facial aesthetics, less is often more, and well-informed decisions are always the most accurate.

    Conclusion

Bichectomy can be a useful tool within a global facial enhancement plan, but it is neither a universal nor harmless treatment, nor a trend to follow without reflection.

If you are considering this procedure, the most important thing is to consult with an experienced facial aesthetics professional who works with a harmonious vision, medical criteria, and respect for naturalness.

Because each face tells its story, and at Puy, we believe the most beautiful thing is that you continue to resemble yourself, only in your best version.
